Manage a dynamic form registration form that adapts to your organization's families needs, while collecting and organizing data effortlessly.

To access the Form Editor, select the Registration Form - New! Module from the OnTrack navigation sidebar.
The registration form will populate automatically from the editor-view, with the sections in exact order as it displayed from the front-facing view.
The registration form is already composed of a standard set of questions listed under the 'Predefined fields' tile. These are not editable, but some of them are removable.
Required questions are noted by an asterisk * and are not removable.
To remove an existing field, click anywhere in the field tile. Field Options will populate on the right sidebar.
If a field is greyed out, it is required and not available to be removed. If it remains in color, you can unselect it and remove it from the registration form.
Underneath the 'Predefined fields' tile of each section, you can add your own unique, organization-specific questions by selecting the '+' button. Fields options will populate on the right sidebar.
There's quite a selection of different types of questions that can be answered, all organization by the options you want to provide.
